What should you know about leg swelling in pregnancy?

Leg swelling in pregnancy is often linked to normal fluid changes and pressure on veins, but sudden or one-sided swelling needs medical assessment. It commonly affects the feet and ankles and may be more noticeable later in the day or after standing.

As the uterus grows, it can slow blood return from the legs. Pregnancy hormones also change how the body handles salt and fluid. Swelling that is gradual, mild, and similar on both sides is often less concerning. Swelling that appears quickly, affects the face or hands, or comes with headache or vision changes can occur with Pre-eclampsia and should be discussed urgently.

Rest on your side, raise your feet when sitting, and avoid standing still for long periods. Gentle walking and ankle movements can support circulation. Wear comfortable shoes and avoid tight bands around the legs. Keep routine antenatal appointments so blood pressure and urine can be checked. Do not restrict fluids or take medication for swelling unless your maternity clinician recommends it.

Contact your maternity team urgently if swelling is sudden, much worse on one side, painful, red, or warm. Get immediate help for chest pain, difficulty breathing, fainting, or coughing blood. A severe headache, visual disturbance, pain under the ribs, or sudden swelling of the face and hands also needs prompt assessment.
