Left-side abdominal pain and ovulation: what to know

Ovulation pain, sometimes called mittelschmerz, can occur when an ovarian follicle stretches and releases an egg. It may be felt on one side of the lower abdomen, including the left, and can switch sides in different cycles. Some people notice a dull ache, while others feel a sharper twinge. It may last for a short time or continue for part of the day. Light spotting or a small amount of vaginal discharge can occur at the same time.

Pain on the left is not specific to ovulation. Bowel irritation, constipation, a urinary problem, an ovarian cyst, menstrual cramps, pregnancy-related problems, or another pelvic condition can feel similar. Digestive symptoms such as diarrhoea, vomiting, bloating, or fever point away from simple ovulation pain and deserve attention. Conditions such as [Diverticular Disease](/conditions/diverticular-disease/) or [Gastroenteritis](/conditions/gastroenteritis/) can also cause abdominal discomfort, but the location and accompanying symptoms matter.

Note when the pain begins, how long it lasts, how strong it feels, and whether it follows a similar pattern in your cycle. Record bleeding, discharge, bowel or urinary changes, nausea, and fever as well. Rest, drink fluids, and try a warm pack over the lower abdomen if heat is comfortable for you. Arrange a routine appointment if the pain keeps returning, interferes with normal activities, does not settle, or feels different from your usual cycle. Seek urgent medical help for sudden or severe pain, pain that is worsening, fainting, marked dizziness, heavy vaginal bleeding, shoulder-tip pain, or a swollen or very tender abdomen. Get prompt assessment if you may be pregnant, have a positive pregnancy test, or have pain with fever, repeated vomiting, pain when passing urine, or blood in your urine or stool. These features can signal a problem that cannot be safely identified from the timing of ovulation alone.
