Short answer

A left lower back spasm is an involuntary tightening that may follow strain or irritation, but weakness, bladder or bowel changes, or severe worsening pain needs urgent care.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common triggers
Lifting, twisting, prolonged sitting, or an unfamiliar workload
Stay mobile
Change position and take short walks within comfort
Urgent warning signs
Bladder or bowel changes, groin numbness, or progressive weakness

Why the back may lock up

A spasm can occur when lower back muscles tighten protectively after lifting, twisting, prolonged sitting, or an unfamiliar workload. The pain may stay on the left or spread across the lower back, with stiffness when standing, rolling in bed, or bending. Muscle Strain and Non-Specific Low Back Pain can have similar features. Pain travelling into the buttock or leg, tingling, or weakness may suggest irritation of a spinal nerve, sometimes associated with a Herniated Disc. Facet Joint Syndrome is another possible source of one-sided back pain.

Note the movement that triggered the spasm, whether pain travels below the knee, and whether coughing or sneezing changes it. These details help a clinician choose an examination and treatment plan.

What may ease it

Keep changing position and take short, easy walks if they are tolerable; prolonged bed rest can make stiffness harder to shift. Use warmth or a wrapped cool pack according to which feels better, and avoid lifting, forceful twisting, and aggressive stretching during the acute phase. When movement is easier, gradual return to normal activity is usually more useful than guarding every motion. A physiotherapist may use exercise, hands-on care, or treatments such as [Myofascial Release](/treatments/manual-and-manipulative-therapy/myofascial-release/) or [Dry Needling](/treatments/physiotherapy-and-rehabilitation/dry-needling/) when appropriate. When to seek care

Seek emergency care for new loss of bladder or bowel control, numbness around the groin, progressive leg weakness, or severe pain after major trauma. Prompt review is needed for fever, unexplained weight loss, pain that is constant at rest, or a history of serious illness. Arrange an appointment if spasms repeatedly return, pain travels down the leg, or ordinary movement remains limited.

Questions for your doctor

Does the pattern fit muscle strain, a facet problem, or nerve irritation? Which movements and exercises are safe now? Do I need imaging? What symptoms would mean I should seek emergency help?

Questions people ask

Should I stay in bed during a back spasm?

Unless a clinician advises otherwise, brief rest is reasonable but gentle position changes and short walks can help prevent worsening stiffness.

Does back spasm always mean a herniated disc?

No. Muscle strain, joint irritation, and non-specific low back pain can also cause spasms; leg symptoms need a closer assessment.
