What one-sided swelling can mean

Pregnancy can cause fluid retention and pressure on veins, often producing swelling in both feet or ankles that is more noticeable later in the day. Swelling mainly in one leg is different and may reflect a blood clot, injury, infection, or another circulation problem. It cannot be diagnosed safely from appearance alone. Pre-eclampsia can present with sudden swelling, particularly when it involves the face or hands, and may occur with headache or visual changes.

Tell your maternity team when the swelling began, whether it is increasing, and whether the calf or thigh is tender. Do not wait for a routine visit if warning symptoms appear.

Until you receive advice, avoid vigorous massage of the swollen area and do not start compression garments or medicines without guidance. If swelling is mild, equal on both sides, and improves with rest, mention it at your next pregnancy check while taking regular movement breaks and elevating your legs when comfortable.

Seek urgent assessment for a leg that is painful, red, hot, markedly larger than the other, or difficult to walk on. Contact your maternity team urgently for severe headache, visual disturbance, pain beneath the ribs, sudden face or hand swelling, or feeling very unwell, as these can accompany pre-eclampsia.
