How it behaves

The iliotibial band is a strong strip of connective tissue running along the outer thigh to the knee. Symptoms commonly appear after a predictable amount of running, downhill walking, or cycling and may settle with rest, then return when the same load resumes. Tenderness is usually felt at the outer knee rather than deep inside the joint. Training increases, worn footwear, hills, limited hip control, or changes in running mechanics can contribute, although one factor rarely explains every case. Other causes of knee pain can feel similar, so an examination matters if the pattern is unusual or persistent.

Building recovery

Pause or scale back the activity that reproduces the sharp outer-knee pain, while choosing comfortable low-impact movement if tolerated. Return to running gradually only when walking and daily activities are comfortable. Physiotherapy can assess hip strength, leg alignment, training load, and movement control. Gait Retraining may adjust cadence, stride, or terrain choices; strengthening should progress rather than provoke a flare. Sports Massage may relieve surrounding muscle tightness for some people but does not replace load management. A Cortisone Injection is a clinician-led option for selected persistent cases, not a substitute for correcting the aggravating pattern.

When to seek care

Book an assessment when pain alters your gait, stops normal exercise, persists after activity changes, or returns each time training resumes. Seek urgent care after a major injury, if the knee locks, gives way repeatedly, becomes hot and swollen, or you cannot bear weight. Calf swelling, chest symptoms, or sudden shortness of breath also require urgent attention.

The band itself is dense connective tissue, so rehabilitation usually focuses on surrounding muscle flexibility, hip strength, and movement control rather than forcing a direct stretch.
