What swelling can tell you
Swelling beside the thumb and wrist after repeated lifting or pinching can occur with De Quervain's tenosynovitis, while swelling at a joint may reflect local irritation. Sudden puffiness after a fall, especially with bruising or reduced movement, needs an injury assessment. If the thumb is one of several swollen joints and stiffness is pronounced after rest, an inflammatory condition such as rheumatoid arthritis is one possibility a clinician may consider. A swollen thumb does not identify its cause by itself.
Protect the thumb
Remove rings or tight jewellery before swelling increases. Rest from forceful pinching, lifting, and repeated phone or tool use. Keep the hand elevated when resting and move the thumb gently only through a comfortable range. A cloth-wrapped cool pack can be used briefly after aggravating activity; protect the skin and avoid pressing on a very tender area. Do not puncture a lump or apply strong massage to a hot, newly injured thumb.
When not to wait
Get urgent care if the thumb is deformed, numb, cold, blue, or impossible to move normally. Same-day advice is appropriate when swelling spreads quickly or comes with fever, marked warmth, or worsening redness. Arrange an examination after a fall or when swelling persists, returns, or limits grip. A clinician may need to examine the wrist as well as the thumb, because nearby injuries can affect both areas.
