What one-sided swelling may mean

A sprain can stretch or tear ankle ligaments and may cause swelling, bruising, and pain after the ankle rolls. A fracture may cause marked pain, bruising, deformity, or difficulty putting weight through the foot; an assessment may be needed even when the injury seemed minor at first.

Inflammation inside the joint can make the ankle feel warm, stiff, or painful. Gout often presents as a sudden, very painful, hot, red, swollen joint, although other joint problems and infection can produce similar features. Osteoarthritis can cause activity-related pain, stiffness after rest, and swelling that builds with use. These patterns overlap, so an examination is more useful than trying to diagnose the cause from swelling alone.

What you can do now

If the swelling followed a mild injury, reduce activities that increase pain and keep the ankle gently moving within a comfortable range. Elevating the leg when resting may reduce the sense of pressure. A cold pack wrapped in cloth can be used briefly at a time; do not place ice directly on skin. Supportive footwear may make walking easier.

Do not force weight-bearing if walking is very painful, and do not massage a newly injured or hot, red ankle. A clinician or pharmacist can help you choose pain relief safely, particularly if you have kidney disease, stomach ulcers, take blood thinners, are pregnant, or have other medical conditions. Arrange an assessment if swelling does not settle, keeps returning, or limits normal walking.

When to seek care

Seek urgent medical care for severe pain, obvious deformity, inability to bear weight, rapidly increasing swelling, or numbness, pale or blue skin, or a cold foot. Prompt assessment is also needed when the ankle is hot and red with fever, a wound, or feeling generally unwell, because infection needs timely treatment.

Get emergency help if ankle or leg swelling occurs with sudden shortness of breath, chest pain, coughing blood, faintness, or new severe weakness. These symptoms can signal a problem beyond the ankle. If none of these warning signs is present but the cause is unclear, book a medical review so the ankle, foot, and circulation can be checked.
