What should you know about lateral ankle swelling?

Short answer
Lateral ankle swelling can follow a sprain, fracture, arthritis, or gout, so its timing, pain, skin changes, and effect on walking help determine the next step.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What may be behind the swelling
Swelling after a twist commonly accompanies an ankle sprain, while immediate severe pain, difficulty bearing weight, or a change in shape can suggest a fracture. Ankle osteoarthritis may cause swelling with activity and stiffness after rest. Gout can produce a sudden, intensely tender, warm, red joint. Swelling may also arise from irritation of nearby tendons or soft tissues.
Compare both ankles and note when the swelling began, whether it followed an injury, and whether the skin is hot or discoloured. A clinician may check the bones, ligaments, joint movement, circulation, sensation, and walking pattern, and may arrange imaging when the examination suggests it.
What you can do initially
After a recent minor injury, protect the ankle, reduce painful loading, elevate it, and use a cold pack wrapped in cloth for short periods. Keep gentle movement within comfort so the joint does not become unnecessarily stiff. Supportive footwear may help once walking is comfortable.
Do not massage a newly injured swollen ankle aggressively or return to sport while walking remains painful. Medication for pain or inflammation is not suitable for everyone; ask a pharmacist or clinician if you have other conditions or take regular medicines.
When to seek medical care
Get urgent assessment if the ankle looks deformed, you cannot bear weight, pain is severe after an injury, or the foot becomes numb, cold, or unusually pale. Prompt care is also needed for rapidly increasing warmth and redness, fever, or feeling unwell. Arrange a clinical review when swelling persists, recurs without a clear injury, or limits normal walking.
Questions for your doctor
Ask which structures are swollen, whether imaging is appropriate, and what activity level is safe. Discuss how to support the ankle, when to begin rehabilitation, and whether a pattern of sudden hot swelling needs assessment for gout or another inflammatory cause.

Questions people ask
Does outer ankle swelling always mean a sprain?
No. A sprain is one possibility; fracture, osteoarthritis, gout, and soft-tissue irritation can produce similar swelling.
When should ankle swelling be checked?
Arrange assessment when swelling is severe, follows significant injury, prevents weight-bearing, becomes hot and red, or does not settle.
