Carbuncles develop when infection involves connected hair follicles and deeper nearby tissue. The area may feel firm, hot, and tender, then develop several drainage points. Redness can extend around it. A carbuncle is different from a single small pimple because the inflammation may be deeper and more extensive.

A clinician will examine the skin and may consider whether a sample is needed, particularly when drainage is present, the infection returns, or treatment has not helped. Conditions that affect immune function can make skin infections harder to control and should be mentioned during the visit.
