Laminectomy, foraminotomy and discectomy explained

A laminectomy removes part of the lamina, the bony plate behind the spinal canal, to create space. A foraminotomy widens the foramen, the passage where a nerve exits the spine. A discectomy removes a portion of disc material when a bulging or herniated disc is pressing on a nerve. Combining procedures can address more than one source of compression at the same level, but the need depends on examination findings and imaging. Spinal stenosis can narrow the canal or nerve exit, while spondylolisthesis can affect stability and influence whether decompression needs to be paired with stabilisation.

These operations aim to relieve nerve pressure; they do not guarantee that every symptom will disappear, particularly when a nerve has been irritated for a long time or pain has another source.

Ask the surgeon to mark which procedure addresses which finding on your scan. Review medicines, allergies, smoking, and other health conditions before admission. Arrange transport and practical help for the early recovery period. After surgery, follow wound instructions and increase walking gradually. Avoid lifting, repeated bending, or twisting until your team gives a personalised plan. Physiotherapy may support posture, walking tolerance, and safe return to daily tasks. Endoscopic spine surgery may be an alternative approach for selected problems, but suitability depends on anatomy and the surgeon’s assessment.

Call the surgical team for fever, increasing wound redness, drainage, a fluid leak, or pain that is worsening rather than settling. Seek emergency care for new bladder or bowel control problems, numbness around the saddle area, rapidly increasing leg weakness, or severe breathing difficulty. New neurological changes should be assessed promptly even if the wound looks normal.
