How lactose can cause pain

When lactase is insufficient, lactose moves into the large bowel rather than being fully digested in the small intestine. Bacteria ferment it and produce gas; extra water may also enter the bowel. Stretching and movement can feel like cramps, pressure, or a dull stomach ache, often alongside bloating and loose stools. The timing after a milk-based food can be a clue, but pain alone does not identify lactose intolerance. Reflux, infection, constipation, irritable bowel syndrome, and other conditions can feel similar.

How to investigate the pattern

Write down the food, portion, time of eating, pain location, pain quality, bowel changes, and how long symptoms last. A clinician may advise reducing lactose for a defined period and then reintroducing it carefully. Smaller portions with meals or lactose-free products may be easier to tolerate. Avoid making several major diet changes at once. If the pattern remains uncertain, food intolerance testing or a broader clinical assessment may be considered. Nutrition counselling is useful if avoiding dairy affects your usual meals.

Pain that should not wait

Seek urgent care for sudden severe pain, a hard or swollen abdomen, faintness, repeated vomiting, blood in stool, black stool, fever with worsening pain, or inability to keep fluids down. Arrange a medical appointment for pain that recurs, wakes you, persists despite avoiding lactose, or comes with weight loss or changed bowel habits. Lactose intolerance should not be used to explain away red flags.
