Short answer

Labia swelling in pregnancy can reflect increased blood flow and pressure, but painful, sudden or one-sided swelling needs review.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Possible contributors
Pelvic blood flow, pressure, fluid retention and vulval veins can all contribute.
Prompt review
Rapid, painful, hot, red or one-sided swelling needs assessment.

Why swelling can happen

Pregnancy increases blood flow to the pelvis and can slow the return of blood from the lower body. Pressure from the growing uterus, fluid retention, prolonged standing and vulval varicose veins can make the labia feel puffy, heavy or uncomfortable. Irritation from soaps, tight clothing or moisture may add soreness. A lump, marked tenderness, warmth, redness, discharge or fever suggests another problem such as infection or a blocked gland and should be assessed. Sudden swelling with a severe headache, visual changes or swelling of the face and hands can be associated with [Pre-eclampsia](/conditions/pre-eclampsia/).

Comfort measures

Wash gently with lukewarm water and avoid scented products, douching and scrubbing. Wear loose, breathable underwear, change out of damp clothing and rest on your side when possible. A cool pack wrapped in cloth may ease discomfort briefly. Avoid pressing, puncturing or trying to drain a lump. Tell your maternity team about the swelling, especially if it is new, worsening or affecting walking, sitting or urination.

When to seek care

Seek prompt advice for swelling that is rapidly increasing, one-sided, very painful, hot or red, or accompanied by fever, pus, unusual discharge or difficulty passing urine. Get urgent help for severe pelvic pain, fainting, heavy bleeding, severe headache, vision changes, pain beneath the ribs or sudden swelling elsewhere. A clinician can examine the skin and glands and decide whether treatment or testing is needed.

Questions for your doctor

Ask whether the appearance fits normal pregnancy-related changes, whether a vulval vein or gland is involved, which hygiene and pain-relief measures are suitable, and what changes should prompt same-day contact with your maternity team.
