Knock knees and foot pronation: what to know

How the structures involved in Knock Knees differ from normal
Illustration: How the structures involved in Knock Knees differ from normal

Short answer

Knock knees and foot pronation can interact during movement, so both knee alignment and foot control should be assessed.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Pronation
An inward rolling and adapting motion of the foot
Assessment
Foot, ankle, knee, hip, and movement are considered together
Possible support
Exercises, activity guidance, footwear advice, or review

The short answer

Knock knees and foot pronation can interact during movement, so both knee alignment and foot control should be assessed. A pronating foot may change how the lower limb loads, but it does not by itself prove that a fixed bone deformity is present.

How the foot and knee connect

Pronation is a normal part of foot motion in which the foot rolls inward and adapts to the ground. When this motion is excessive or poorly controlled, the ankle, shin, and knee may move inward during standing, walking, or running. That moving pattern can resemble knock knees even when the underlying bones are aligned differently.

A clinician may compare barefoot and shod walking, observe a single-leg squat, check hip strength, and examine the arch, ankle, and knee. Pain may occur around the kneecap, shin, foot, or ankle, but the location alone does not identify the source. The aim is to find which movements are modifiable and which are structural.

Helpful next steps

Notice whether the knees move inward during particular tasks and whether shoes, surfaces, or fatigue change the pattern. Avoid forcing the feet into a rigid position or buying corrective products based only on photographs. An assessment can determine whether footwear advice, exercises, temporary support, or simple observation is appropriate.

Physiotherapy may address calf flexibility, hip and leg strength, balance, and control of the foot during loading. See [Paediatric Physiotherapy](/treatments/physiotherapy-and-rehabilitation/paediatric-physiotherapy/) when the patient is a child or adolescent. If the alignment is fixed and symptoms remain significant, an orthopaedic specialist may discuss options such as [Osteotomy](/treatments/reconstructive-orthopedic-surgery/osteotomy/).

When to seek care

Seek an assessment for persistent activity-related pain, recurrent ankle or knee problems, limping, rapid change, marked asymmetry, or difficulty with normal walking. Prompt medical advice is appropriate after an injury that causes swelling or prevents weight-bearing. A pronated foot without symptoms may not require treatment.

Questions for your appointment

Ask whether the inward movement comes mainly from the foot, shin, hip, or bone shape. Discuss whether exercises or footwear changes are indicated, how to judge improvement, and what symptoms would justify imaging or specialist review.

Questions people ask

Does pronation cause every case of knock knees?

No. Pronation can influence dynamic leg movement, while some knock-knee alignment comes from the bones or other parts of the limb.

Are insoles always needed?

No. Insoles are considered only when an examination shows they may improve comfort or control for that person.
