Knee swollen and painful for no reason: what to know

Short answer

A knee that becomes swollen and painful without an obvious injury needs attention because inflammation, fluid buildup, or an unrecognised joint problem may be involved.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Track
Onset, heat, movement limits, and episodes of giving way
Avoid
Impact, deep bending, and kneeling while pain is active

What the combination can mean

Swelling may come from fluid inside the joint or irritated tissue around it. Gout can trigger sudden inflammation; knee bursitis affects a small cushioning sac; and osteoarthritis can cause aching with activity and intermittent swelling. A Baker's Cyst may create fullness behind the knee. A meniscus tear or ACL tear can sometimes occur with a twist that seemed minor at the time. Chondromalacia Patellae and Patellar Tendinitis more often cause pain around the kneecap, especially with stairs, squatting, or repeated loading.

The pattern matters: note whether the swelling appeared quickly, whether the knee feels hot, whether it catches or gives way, and whether you can fully bend and straighten it.

What you can do now

Reduce activities that increase pain, particularly running, jumping, deep squats, and kneeling. Rest the leg in a comfortable position, use a cold pack wrapped in cloth for short periods, and raise the leg when resting. Gentle movement within a comfortable range can help prevent stiffness; do not force a painful bend. Arrange a clinical assessment if the swelling does not settle, keeps returning, or limits walking. A clinician may examine the joint and decide whether imaging or fluid testing is useful.

When to seek care promptly

Seek urgent medical care if the knee is hot and very tender with fever or chills, if swelling appeared rapidly after an injury, if you cannot bear weight, or if the joint is locked. Get prompt help for marked calf swelling, chest symptoms, or new shortness of breath. These signs need assessment because infection, a significant injury, or a circulation problem may require timely treatment.

Questions to take to an appointment

Ask what the location and type of swelling suggest, whether a meniscus or ligament problem needs checking, and what activity is safe while the knee settles. You can also ask whether medication is appropriate for you and what changes should trigger an earlier review.

Questions people ask

Can knee swelling happen without a clear injury?

Yes. Inflammation, crystal irritation, arthritis, or an unnoticed twist can produce swelling even when no major accident occurred.

Should I keep walking on a swollen knee?

Limit walking to what feels manageable and avoid activities that increase pain. Seek assessment if weight-bearing is difficult or the swelling persists.
