Why a swollen knee may feel warm

Warmth can occur when the body sends more blood and inflammatory activity to the area. Gout commonly causes an acutely inflamed joint, and knee bursitis can make a superficial area tender and warm. Knee osteoarthritis may also flare with swelling. A Baker's Cyst can cause pressure behind the knee, while a meniscus tear or ACL tear may cause fluid after a knee event. Infection is less common but important to consider, especially when warmth is spreading, the skin is red, or you feel unwell. A warm knee does not by itself identify the condition.

How to respond

Stop strenuous exercise and avoid kneeling or deep squats while the knee is hot and swollen. Support the leg when resting and apply a wrapped cool pack briefly, allowing the skin to return to normal between applications. Do not massage a red, very tender area or use leftover antibiotics. Keep track of fever, redness, pain, and your ability to bend or bear weight. A clinician may need to examine the knee and may consider Joint Aspiration to evaluate or relieve excess fluid.

When warmth needs urgent attention

Get urgent care if the knee is hot and rapidly becoming more painful, you have fever or chills, redness is spreading, you cannot move the joint, or you cannot safely walk. Seek immediate help for severe swelling with breathlessness or chest pain. If warmth and swelling remain without these warning signs, arrange a prompt appointment because persistent inflammation still needs a diagnosis.
