Short answer

Knee swelling without a remembered injury can come from inflammation or fluid build-up and should be assessed if it persists or recurs.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Observe
Onset, one-sided or both-sided swelling, warmth, redness, stiffness, and locking.
Protect
Reduce provoking load while keeping comfortable movement.
Arrange review
Persistent, recurrent, hot, rapidly increasing, or function-limiting swelling.

Possible explanations

Not every knee problem begins with a fall or twist. Repetitive loading can irritate tissues gradually, and knee osteoarthritis may cause fluid and stiffness after activity. Knee Bursitis tends to create swelling near a specific pressure point. Gout can cause a sudden inflamed joint, often with marked tenderness and warmth. A Baker's Cyst may feel like fullness behind the knee. A meniscus tear or ACL tear can occasionally be linked to a movement that seemed minor or was not noticed at the time.

Record when you first noticed the change, whether one or both knees are affected, and any warmth, redness, fever, stiffness, locking, or instability. A clinician may compare both knees, assess movement, and decide whether imaging or fluid testing is needed.

Practical next steps

Reduce activities that increase the swelling, but keep the knee gently moving within comfort. Elevation during rest and a cloth-wrapped cold pack may help symptoms. Avoid kneeling or prolonged squatting if these increase pressure. Do not start a new exercise programme while the cause is unclear, and do not take someone else’s medication.

Arrange a medical review if swelling remains or returns. Bring a list of medicines and any history of gout, joint disease, recent infection, or unusual exertion. If fluid is confirmed, joint aspiration may sometimes help diagnosis or relieve pressure.

When to seek care

Seek urgent care for a hot or red knee with fever, severe sudden pain, inability to bear weight, a deformity, or new calf swelling and pain. Prompt review is also appropriate when the knee locks, repeatedly gives way, or becomes rapidly more swollen. If the swelling is mild but lasts or disrupts daily activity, book a routine assessment rather than waiting for another flare.

Questions for your doctor

Which examination findings could explain swelling without a clear injury? Should I be checked for gout, bursitis, osteoarthritis, a Baker's Cyst, or a meniscus problem? What activity is safe while the knee is swollen? Would imaging or joint aspiration clarify the cause?

Questions people ask

Can a knee swell without an injury?

Yes. Inflammation, gradual joint irritation, bursitis, gout, or fluid behind the knee can cause swelling without a clear accident.

Is unexplained knee swelling always serious?

Not always, but persistent or recurrent swelling needs assessment because its cause cannot be identified from appearance alone.
