Knee Swelling in Pregnancy: What It Can Mean and What to Do

Short answer
Knee swelling in pregnancy can come from fluid retention and joint strain, but sudden or marked swelling needs medical assessment.

What knee swelling can mean
Pregnancy changes circulation, shifts your centre of gravity and places more load on the knees. Fluid can collect in the lower legs, especially after standing, travelling or being active. A knee can also swell after a twist, overuse or irritation around the joint. Swelling on both sides that builds gradually is different from one knee becoming hot, red and painful.
Swelling with a severe headache, vision changes, pain below the ribs or sudden swelling of the face and hands can be a warning sign of pre-eclampsia. Learn more about [Pre-eclampsia](/conditions/pre-eclampsia/).
Practical steps
Rest the knee between activities and raise your legs when sitting. Gentle movement, such as short walks or ankle pumps, can support circulation; stop if pain increases. Wear comfortable, supportive shoes and avoid standing in one position for long periods. A cool pack wrapped in cloth may ease discomfort for a short time. Do not start anti-inflammatory medicine, compression garments or supplements without checking that they are suitable in pregnancy.
When to seek care
Contact your maternity team promptly if swelling appears suddenly, affects one leg, or comes with warmth, redness, calf pain, breathlessness or chest pain. Seek urgent help for severe breathlessness, fainting or chest pain. Arrange a clinical review if the knee remains swollen, locks, gives way, follows an injury or makes walking difficult. Mention any new headache, visual disturbance or upper-abdominal pain without waiting for a routine visit.
Questions for your doctor
Ask whether the pattern suggests fluid retention, an injury, a joint problem or a circulation concern; whether you need an examination or scan; and which pain-relief options are suitable at your stage of pregnancy.
