What to know about knee swelling at night

Knee swelling that becomes noticeable at night can reflect joint fluid, irritation, or a condition that needs assessment. The knee may look fuller after walking, standing, exercise, or work during the day. Lying still can also make stiffness and pressure easier to notice. Night swelling alone does not identify one diagnosis.

What night-time swelling can mean

Fluid may collect inside the knee when the joint lining is irritated. Possible explanations include knee osteoarthritis, bursitis, gout, a meniscus tear, or an ACL tear. A Baker's cyst can create fullness behind the knee and may feel more obvious when the leg is bent. The pattern matters: warmth, redness, sudden pain, locking, instability, or swelling after a twist can guide the clinician's examination.

Keep a short record of when swelling appears, whether it is present in the morning, what activity preceded it, and whether the knee feels hot or unstable. This helps distinguish an activity-related flare from swelling that is continuing regardless of use.

What you can do tonight

Reduce activities that increase pain and rest the leg in a comfortable position. Elevating the lower leg may ease pressure, and a wrapped cold pack can be used briefly if cold is safe for you. Avoid pressing firmly on a tender lump behind the knee. Do not restart running, jumping, or twisting activities until movement is comfortable and the knee is assessed if swelling persists.

Arrange a clinical review if night swelling keeps returning, limits sleep, or does not settle with rest. An examination may be followed by imaging or, in selected cases, joint aspiration to clarify what is causing the fluid.

When to seek care promptly

Seek urgent medical care if the knee is suddenly very swollen, hot, red, or severely painful; you have fever; you cannot bear weight; the knee is locked; or the leg becomes numb, pale, or unusually cold. Sudden calf swelling or breathlessness also needs urgent attention. These signs can indicate an infection, significant injury, or a circulation problem and should not be managed at home.
