What should you know about knee swelling after ACL surgery?

Short answer
Knee swelling after ACL surgery often reflects surgical healing, but a sudden increase or swelling with fever or calf symptoms needs review.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Recovery clue
- A changing pattern after exercise should be discussed if it does not settle with the prescribed rest plan.
- Call promptly
- Fever, wound drainage, increasing redness, or new calf symptoms.
Why swelling happens
The graft, tunnels, incision, and surrounding tissues need time to settle. Fluid inside the joint can restrict bending and make the knee feel full. Swelling may vary with exercises, walking, or leaving the leg down for a long time, so compare the overall trend rather than one moment.
The original ACL tear may have occurred with a meniscus tear or other injury, and these can influence recovery. A swollen knee can also have unrelated causes such as gout, bursitis, osteoarthritis, or a Baker's cyst, so do not assume every change comes from the operation.
Supporting recovery
Use the surgeon's plan for brace settings, crutches, exercises, elevation, and cold therapy. Work on the prescribed range of motion without forcing a painful bend. Keep the wound protected and take only medicines recommended for you.
Tell your physiotherapist if swelling changes your quadriceps control or prevents the planned exercises. The surgical team can decide whether examination, imaging, or joint aspiration is appropriate; do not puncture the knee or alter rehabilitation independently.
When to call
Call the surgical team for increasing redness, wound drainage, fever, chills, worsening pain, or swelling that is not following the expected recovery pattern. Seek urgent care for marked calf pain or swelling, sudden breathlessness, chest pain, fainting, or a cold, pale, numb, or weak foot.
Useful questions
Ask how your swelling compares with the expected course, when to use ice and elevation, which movement is safe, and whether your calf or wound needs an examination.

Questions people ask
Can physiotherapy make the knee swell more?
It can temporarily increase fullness or stiffness. Your therapist can adjust the load when swelling interferes with movement or persists.
Is swelling proof that the ACL graft has failed?
No. Swelling has many explanations. Stability, examination findings, injury history, and recovery progress are needed to assess the graft.
