Knee replacement swelling after 2 years: what to know

A knee may swell after a long walk, unfamiliar exercise, kneeling, or a sudden increase in activity. Swelling can also reflect irritation in the soft tissues, inflammation inside the joint, fluid behind the knee, or a problem involving the implant or surrounding bone. The pattern matters: note whether it appears after activity, remains at rest, comes with warmth, or is linked with reduced movement. Knee swelling can accompany conditions such as knee bursitis, gout, a Baker's cyst, or a meniscus tear; these possibilities do not confirm a diagnosis in someone with a replacement.

A clinician may examine your scar, range of motion, stability, temperature, and walking pattern. They may review earlier imaging and decide whether new imaging or fluid testing is appropriate.

Record when the swelling began, what makes it worse, and whether pain, stiffness, clicking, fever, or discharge is present. Reduce activities that clearly provoke it while continuing gentle movement within comfort. Elevating the leg after activity may help; use a cold pack wrapped in cloth for short periods if your care team has allowed it. Do not self-start antibiotics or anti-inflammatory medicines without checking for kidney, stomach, blood-thinning, or other safety concerns. An assessment may include joint aspiration when a clinician needs to examine fluid or relieve pressure.

Arrange prompt medical review for swelling that is persistent, repeatedly returning, or accompanied by increasing pain, warmth, redness, or loss of movement. Seek urgent care for fever with a hot or rapidly enlarging knee, wound drainage, sudden inability to bear weight, severe calf swelling or pain, chest pain, or breathlessness. These warning signs need direct assessment and should not be explained away as normal recovery from an older operation.
