What should you know about keep having back spasms?

Short answer

Recurring back spasms deserve assessment because muscle strain, facet joint pain, disc irritation, and other problems can produce them.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why spasms may keep returning

A back spasm is a sudden tightening of muscle that can make movement feel stuck or sharply painful. Repeated episodes may follow overuse, an awkward lift, reduced conditioning, or a previous injury. A muscle strain can present with spasm, while facet joint irritation or a herniated disc may also produce back pain and protective muscle tightening. Non-specific low back pain is another possible clinical description when no single structure explains the symptoms.

A spasm alone cannot identify the cause. The location of pain, spread into the leg, numbness, weakness, fever, injury history, bladder or bowel changes, and the effect of rest or movement help guide assessment.

What to do now

During a flare, change position gently, avoid heavy lifting and forceful twisting, and use a comfortable level of movement rather than prolonged bed rest. Note triggers, duration, pain spread, and recovery between episodes. A clinician or physiotherapist can build a plan around trunk strength, hip movement, lifting technique, and work or sport demands. Options such as [Myofascial Release](/treatments/manual-and-manipulative-therapy/myofascial-release/) or [Dry Needling](/treatments/physiotherapy-and-rehabilitation/dry-needling/) should be considered only after an appropriate assessment. See [Back Muscle Spasm](/symptoms/back-muscle-spasm/) for symptom-focused information.

When to seek care

Seek urgent assessment for new loss of bladder or bowel control, numbness around the groin, progressive leg weakness, or difficulty walking. Get prompt medical care for fever with back pain, unexplained weight change, severe pain after significant trauma, or pain that is rapidly worsening. Recurrent spasms without these warning signs still merit an appointment if they limit sleep, work, exercise, or normal movement.

Questions for your doctor

What features point toward muscle strain, facet joint syndrome, disc irritation, or non-specific pain? Do I need an examination or imaging? Which movements should I practise or avoid? What changes mean I should seek urgent help?

Questions people ask

Do repeated spasms prove I have a herniated disc?

No. A herniated disc is one possible cause, but muscle strain, facet joint pain, and non-specific low back pain can also cause spasms.

Should I stay in bed during a spasm?

Usually, gentle position changes and tolerable movement are preferable to prolonged bed rest, unless a clinician gives different advice.
