What Katayama fever means

Katayama fever describes an immune reaction that can occur after a person becomes infected with schistosome parasites. The relevant exposure is contact with untreated freshwater in places where the parasite's life cycle occurs; ordinary drinking-water exposure is not the usual route. Symptoms can include fever, marked tiredness, headache, muscle aches, cough, abdominal discomfort, diarrhoea or an itchy rash.

The illness can resemble common viral infections and other travel-related conditions. Fever alone does not establish the diagnosis. A clinician will consider the timing of symptoms, travel route, freshwater contact, examination and tests. Mention exposure even if it happened weeks earlier.

What to do after a possible exposure

Arrange medical advice promptly if fever or an unexplained flu-like illness follows freshwater contact during travel or residence in an affected area. Write down dates, locations, swimming or wading activities, symptoms and medicines taken. Avoid further freshwater exposure until assessed. Do not start antiparasitic treatment without a clinician, because timing and confirmation can affect the plan.

When fever is urgent

Seek urgent care for confusion, severe weakness, breathing difficulty, a seizure, fainting, stiff neck, repeated vomiting, dehydration or a rapidly worsening condition. A child, pregnant person or person with reduced immunity should receive medical advice early. Persistent fever after travel also deserves assessment, even when symptoms seem mild.

Explain where you travelled, whether you swam or waded in freshwater, when symptoms began and whether you noticed a rash or breathing symptoms. Ask which blood or other tests are needed, whether follow-up testing is necessary, and what warning signs should send you back urgently.
