How jaw pain reaches the ear

Jaw clenching, teeth grinding, muscle strain, or irritation of the temporomandibular joint can cause aching in front of the ear, tenderness in the cheek, clicking, stiffness, or limited opening. The ear may feel blocked even when its examination is normal. Earwax buildup and middle-ear infection are separate causes that can produce ear pain, hearing change, pressure, or discharge. A cold, sinusitis, or tonsillitis may also refer discomfort toward the ear. A clinician may need to examine both the ear and jaw rather than assuming one source.

Steps that may ease it

Use a soft diet temporarily and avoid gum, hard foods, nail biting, and opening the mouth very wide. Notice whether you clench during concentration or sleep, and keep the teeth gently apart when resting. A warm compress over the jaw muscles can be soothing. Use pain medicine only as directed on its label, taking advice first if you have other medical conditions or take regular medicines. Do not insert anything into the ear or start antibiotics without an assessment.

When to seek care

Book an appointment when pain persists, recurs, disrupts sleep, or limits eating or mouth opening. Seek prompt care for fever, ear discharge, new hearing loss, facial swelling, or severe tenderness. Emergency assessment is needed for breathing or swallowing difficulty, rapidly increasing swelling, facial weakness, confusion, or jaw pain accompanied by chest pressure or breathlessness.
