IUGR at the 5th percentile: what should you know?

The 5th percentile is a chart position based on an ultrasound estimate of fetal size. It suggests the baby is smaller than most babies at the same pregnancy stage, yet it cannot distinguish a naturally small baby from fetal growth restriction on its own. The clinician will compare the result with previous measurements and examine abdominal growth, amniotic fluid, placenta-related blood flow and fetal activity. A falling growth pattern or abnormal blood flow may carry a different significance from a stable pattern with reassuring wellbeing checks.

Follow the appointment schedule provided by your obstetric or midwifery team. A repeat growth scan can show whether the baby is continuing to grow, and fetal heart monitoring can add information about immediate wellbeing. Take your pregnancy notes to appointments and write down the movement pattern you usually notice. Discuss blood pressure and other maternal health concerns with the team, since they may affect monitoring. Birth timing is individual: the team weighs all findings rather than using the 5th percentile as a standalone rule.

Contact maternity services urgently if movements are reduced or different, you have bleeding or fluid leakage, regular painful contractions, severe abdominal pain, a severe headache, vision changes or sudden swelling. Go for urgent assessment if you feel acutely unwell. These symptoms deserve prompt review regardless of whether the scan result later proves to be stable.

Ask which measurement produced the 5th percentile and how reliable the estimate is. Ask whether the growth trend, fluid and Doppler results are reassuring, when the next scan is due, and how you should respond to movement changes. Ask what would prompt closer monitoring, hospital assessment, fetal heart monitoring or a discussion about induction of labour.
