IUGR at the 3rd percentile: what should you know?

A percentile compares an estimated fetal measurement with measurements from pregnancies at the same stage. The 3rd percentile is a very small measurement on that chart, but it is not a diagnosis about the baby’s future health. The scan estimate has limits, and one result is less informative than the pattern across time. Clinicians also review abdominal growth, head and limb measurements, placenta-related Doppler blood flow, amniotic fluid and fetal movements. A small baby may be healthy, while restricted growth can reflect reduced placental function.

Attend the surveillance appointments your maternity team schedules. These may include a growth scan to assess change, Doppler studies and fetal heart monitoring to evaluate current wellbeing. Keep a clear record of the movement pattern that is normal for your baby and report a meaningful reduction promptly. Your team may discuss birth planning if monitoring becomes concerning or if continuing the pregnancy appears less safe. The percentile alone does not determine whether labour should be induced or whether a caesarean birth is needed.

Call your maternity unit without delay for reduced or changed fetal movements, vaginal bleeding, suspected waters breaking, painful regular contractions, severe abdominal pain, severe headache, visual disturbance or sudden swelling. Seek urgent help as well if you feel seriously unwell. These warning signs require assessment independently of the percentile, so do not wait for the next scheduled scan.

Ask what measurement was at the 3rd percentile and whether other measurements agree. Ask whether blood-flow and fluid findings are reassuring, when the next growth scan or fetal heart monitoring is due, how movements should be assessed, and what specific findings would alter the birth plan. Ask whether induction of labour is relevant now or only if monitoring changes.
