What should you know about IUD spotting after sex?

Light bleeding after penetration can occur when the vagina or cervix is irritated, especially with dryness or friction. An IUD may make the timing noticeable, but spotting is not proof that the device has moved. Cervical inflammation, Chlamydia, cervical dysplasia, polyps, and fibroids can also cause bleeding after sex. Less common but important causes include cervical cancer or womb cancer. Bleeding after sex is a presentation of several conditions; it does not by itself tell you which condition is present.

Record whether the blood is pink, red, or brown, how long it lasts, and whether it occurs only after sex. Mention pelvic pain, unusual discharge, fever, missed periods, or pregnancy possibility, as these details affect the urgency and testing plan.

Use lubricant, avoid penetration until bleeding and discomfort settle, and arrange a medical review if the symptom repeats or has no clear explanation. Do not remove the IUD yourself. A clinician may examine the vagina and cervix, check the IUD strings, offer pregnancy and infection testing, and advise whether cervical screening or imaging is needed.

Seek urgent care for heavy bleeding, severe pelvic pain, fainting, fever, weakness, or a positive pregnancy test with pain or bleeding. Contact a clinician promptly when spotting keeps recurring, occurs after menopause, or comes with foul-smelling discharge. These patterns need timely assessment rather than being attributed to the IUD alone.

Ask whether the cervix and IUD position should be examined, whether infection or pregnancy testing is needed, and whether your cervical screening is up to date. Ask what amount of bleeding should prompt urgent care.
