Itchy swollen lip: what to know

The timing can offer clues. New lip balm, lipstick, toothpaste, food, medicine, or dental material can irritate or trigger a contact reaction. A sudden, deeper swelling without much surface rash can occur with angioedema. Itching, hives, facial swelling, or swelling in the mouth may point to a broader allergic response. Crusting, warmth, tenderness, or a sore can suggest irritation or infection. One symptom alone cannot identify the cause.

Note when the swelling began, whether one or both lips are involved, what touched the area, and whether it is changing. Avoid assuming that a swollen lip has one specific cause.

Stop any newly introduced lip or oral product and rinse gently with lukewarm water. Do not scratch, squeeze, or apply several new creams at once. A cool, clean compress may ease itch and puffiness. Keep a record of foods, medicines, products, and insect exposures around the onset. If symptoms keep returning, a clinician can review the pattern and decide whether allergy assessment or another examination is appropriate.

Get emergency help immediately for trouble breathing, wheezing, throat or tongue swelling, difficulty swallowing, faintness, confusion, or rapidly spreading swelling. Seek prompt medical assessment for fever, severe pain, pus, marked warmth, a spreading rash, or swelling that continues to worsen. A clinician should also assess repeated episodes or swelling that does not settle.

Ask what pattern best fits the swelling, whether a recent product or medicine could be responsible, which warning signs require emergency action, and how to prevent another episode. Ask whether the episode could be related to angioedema.
