Itchy red rash in the groin: what to know

Moist, tender redness in a fold may occur with intertrigo, especially when heat and rubbing are involved. A scaly border that extends outward can fit ringworm. New underwear, detergent, wipes, pads, lubricants, or shaving products can trigger contact dermatitis. A rash that is very itchy at night, or affects close contacts, raises the need to assess for scabies. Atopic dermatitis, psoriasis, heat rash, and drug rash are other possibilities. The symptom describes a presentation; it does not identify one condition by itself.

Clean gently with lukewarm water, dry the folds carefully, and choose loose breathable clothing. Pause new fragranced products and avoid shaving, scratching, and sexual contact if the skin is sore or open. Keep towels and clothing personal until an infectious cause is excluded. Avoid layering multiple creams or using a steroid alone on an undiagnosed groin rash, because this may worsen or disguise some conditions. Photograph the rash and record its timing, spread, and exposures.

Get prompt assessment if the redness spreads quickly, the area becomes hot or very painful, pus or fever appears, or walking is difficult. Seek urgent help for severe swelling, open sores with significant pain, breathing difficulty, facial swelling, or faintness. Arrange an appointment for a recurrent rash, persistent itch, possible sexual exposure, or symptoms affecting a partner or household member.
