Itchy rash under the testicles: what should you know?

Sweat and rubbing can inflame the fold, sometimes called Intertrigo. A scaly border extending onto the inner thigh may suggest Ringworm. Contact Dermatitis can follow a new soap, lubricant, detergent, condom material, or grooming product. Atopic Dermatitis or Psoriasis can cause recurring itch and scale. Heat Rash may follow heavy sweating. Scabies is worth considering if night-time itch is severe or close contacts are also affected. A medicine-related widespread eruption may be Drug Rash.

Rinse with lukewarm water, use a mild fragrance-free cleanser if needed, and pat dry rather than rubbing. Wear loose underwear and change it after sweating. Stop new products on the area and avoid scratching, shaving, or applying perfumed powders. Keep creams away from the urethral opening and broken skin unless a clinician advises otherwise. Because steroid and antifungal treatments are not interchangeable, obtain advice if the cause is unclear.

Seek prompt care for severe pain, sudden swelling of the testicle, fever, pus, ulcers, blisters, painful urination, or rapidly spreading redness. Arrange a dermatology or primary-care review when the rash persists, recurs, reaches the penis, or does not improve after removing an irritant. Sudden testicular pain or swelling needs urgent evaluation rather than home treatment.

Ask whether the rash is in the fold or on the testicular skin itself, whether a fungal test is useful, which products and medicines are safe there, and whether a sexual-health assessment is appropriate for your symptoms.
