Itchy rash turning purple: when to get help

Purple or dusky areas can occur when blood has leaked beneath the skin, when inflammation is severe, or when a rash is changing as it heals. Scratching can bruise skin, and some medicines can trigger Drug Rash or alter bleeding risk. Contact Dermatitis, Atopic Dermatitis, and other inflammatory conditions may become darker after irritation, but colour alone cannot distinguish a bruise from a more urgent process. Note whether the spots are flat or raised and whether a clear glass pressed against them makes them fade.

Take a clear photograph in good light, note when the purple colour appeared, and list medicines or supplements you use. Avoid scratching, vigorous rubbing, and starting a new cream before assessment. If you are otherwise well, contact a clinician promptly for advice; keep the affected area protected and seek help sooner if it is enlarging or becoming painful.

Seek emergency help for purple spots with fever, confusion, faintness, severe headache, stiff neck, breathing trouble, facial swelling, bleeding elsewhere, severe abdominal pain, or rapidly worsening illness. Urgent assessment is also needed for a painful purple rash, blistering, skin peeling, or spots that do not fade with pressure, especially when they are spreading.

Ask whether the colour suggests bruising, bleeding beneath the skin, infection, inflammation, or a medicine reaction; mention every medicine and supplement and ask whether examination or blood tests are needed.
