Itchy Rash on Hands: Possible Causes and Next Steps

Frequent handwashing, cleaning chemicals, solvents, and wet work can damage the skin barrier and cause irritant contact dermatitis. Gloves, fragrances, metals, and rubber additives may trigger allergic contact dermatitis. Tiny very itchy blisters on the sides of fingers can occur with hand eczema. Thick scale on knuckles or palms may occur with psoriasis. A one-sided scaly edge can suggest ringworm, while intense night itch with affected contacts can suggest scabies. Cracks, crusting, or drainage may indicate infection.

Use lukewarm water and a gentle fragrance-free cleanser, then moisturise immediately after drying. Wear protective gloves for cleaning, with cotton liners if sweating makes symptoms worse; replace wet gloves promptly. Avoid picking blisters or peeling scale. Keep a list of soaps, sanitiser, gloves, tools, and new medicines, and photograph the hands during a flare. A clinician may need to examine the palms, finger webs, nails, and wrists together.

Seek urgent care for rapidly increasing swelling, severe pain, fever, pus, red streaks, loss of movement, or a rash with facial swelling or breathing trouble. Get prompt advice if blisters are widespread, skin is peeling, or symptoms started after a new medicine. Arrange an appointment when cracks bleed, sleep is disturbed, work is affected, or careful skin protection does not settle the rash.
