Itchy rash in the male genital area: what to know

The genital area is exposed to sweat, occlusion, friction, and products that can irritate delicate skin. Contact dermatitis may follow soap, lubricant, condoms, or laundry products. Eczema and psoriasis can cause itch and scaling. Ringworm more often affects the groin crease and inner thigh, while scabies can cause marked night-time itching and itchy contacts. Blisters, ulcers, discharge, or burning urine may point to a condition that needs sexual-health assessment; a rash alone cannot identify its cause.

Wash with lukewarm water or a gentle fragrance-free cleanser, then pat dry. Wear loose underwear, change after sweating, and stop newly introduced products. Avoid scratching, sexual contact that causes pain, and applying antiseptics or steroid creams without advice. Do not share towels. If there has been a new sexual exposure or there are sores or discharge, arrange testing rather than trying to diagnose the rash from appearance.

Seek urgent help for severe genital pain, rapidly increasing swelling, inability to urinate, fever with worsening redness, skin colour change, facial swelling, or breathing difficulty. Arrange prompt care for blisters, ulcers, pus, discharge, painful urination, or a rash that is spreading or not settling. A clinician can inspect the skin and choose swabs or other tests when indicated.

Ask whether the rash is more consistent with irritation, eczema, ringworm, scabies, or a sexually transmitted infection; whether testing is needed; when sexual activity is safe; and which treatment is suitable for genital skin.
