Itchy rash in the male groin: what to know

A rash in the crease between the thigh and body may reflect intertrigo from moisture and rubbing. A scaly advancing edge on the inner thigh can fit ringworm. Irritation may follow new underwear, detergent, shower gel, lubricant, condoms, or shaving. Atopic dermatitis or psoriasis can affect the groin and may involve other areas. Scabies can cause night-time itching and may affect household or intimate contacts. The penis and scrotum can be irritated by different causes, so location matters.

Rinse gently, dry carefully, and change out of damp clothing. Choose loose, breathable underwear and avoid fragranced products or shaving until the skin recovers. Keep personal towels separate and wash clothing regularly. Avoid mixing several creams, especially steroid creams, because they can mask or worsen a fungal infection. A clinician or pharmacist can assess whether an antifungal, anti-inflammatory, or other treatment fits the appearance.

Seek urgent care for severe scrotal pain or swelling, rapidly spreading redness, fever, pus, tissue colour change, difficulty urinating, or a rapidly worsening illness. Arrange a medical review for blisters, ulcers, discharge, pain with urination, a new sexual exposure, or a rash that persists or returns. These features can require examination and targeted testing.

Ask what the location and border suggest, whether testing for ringworm or another infection is needed, which products to stop, whether partners or household contacts need advice, and how to prevent another flare.
