What should you know about itchy rash after fever?

Some viral illnesses cause a rash as the fever settles. Chickenpox often produces very itchy spots that become fluid-filled blisters and then scab, while a common cold or COVID-19 can occur with broader viral symptoms and an occasional rash. A new medicine can also trigger hives or a widespread rash; record the name and the time it was started. Redness that is hot, tender, rapidly spreading, or associated with swelling may indicate cellulitis rather than a simple post-viral change. In children, breathing noise, a barking cough, or marked sleepiness needs prompt attention. The rash pattern alone cannot confirm its cause.

Take a clear photograph in good light and note when the fever ended, where the rash began, and whether it is spreading. Keep the skin cool, use loose clothing, and avoid scratching; short nails can reduce skin injury. Drink fluids and follow the label for any age-appropriate medicine. Do not start or stop a prescribed medicine without speaking with a clinician, especially if the rash began after it. Avoid close contact with vulnerable people until an infectious cause has been considered.

Get emergency help for breathing difficulty, throat or facial swelling, collapse, confusion, a seizure, or a rash that does not fade when pressed with a clear glass. Seek same-day assessment for a very unwell child, severe headache with neck stiffness, purple or bruise-like spots, blistering, skin peeling, painful eyes or mouth, or a rapidly spreading hot rash. Arrange review if the fever returns or the rash persists or worsens.
