Itchy Dark Spots on the Legs: What to Know

A mark that remains after a bite, scratch, or rash may be post-inflammatory hyperpigmentation. Sun-exposed flat spots can have a different pattern from patches that are scaly, spreading, or lighter in the centre. Melasma and sun spots are usually considered in the right distribution, while tinea versicolor more often affects the trunk but can alter skin colour. The itch, scale, border, timing, and number of spots help a clinician narrow the possibilities.

Use a gentle cleanser, moisturise dry skin, and protect exposed legs from sunlight. Avoid picking, harsh scrubs, and unlabelled bleaching mixtures. Take a clear photo when the spot is active and note new soaps, plants, medicines, travel, or bites. A clinician can decide whether treatment should target inflammation, infection, or residual colour. Chemical peels, skin brightening treatment, or picosecond laser treatment may be considered only after the diagnosis and skin sensitivity are reviewed.

Seek prompt care if spots appear with facial swelling or breathing difficulty, fever, rapidly spreading redness, warmth, pus, or severe pain. Book a dermatology assessment for a spot that changes quickly, bleeds, forms a sore, or does not improve, especially if it looks unlike your other marks.
