What should you know about itching with rash in pregnancy?

Reading the rash pattern

Dry, cracked, scaly areas can fit eczema or irritant dermatitis, while a rash after a new soap, fabric, cream, or jewellery may suggest contact dermatitis. Raised fleeting welts are typical of hives. Moist red patches in skin folds can be infected or irritated, and scabies may cause intense night-time itch with small bumps or lines, sometimes affecting household members. A pregnancy-related eruption may begin in abdominal stretch marks and spread, but a rash can vary widely in appearance.

A rash with fever, blisters, sores, facial swelling, or involvement of the eyes or mouth needs prompt assessment. The rash itself does not establish its cause, and pregnancy changes which medicines are suitable.

Steps that may reduce irritation

Use lukewarm water, a fragrance-free cleanser, and a bland moisturiser. Wear loose cotton clothing and avoid overheating. Apply a cool damp cloth for short periods and keep nails short to limit skin breaks. Stop newly introduced products until a clinician advises otherwise, but do not stop prescribed pregnancy medicines without checking. Photograph the rash in good light and record where it started, how quickly it spread, and any exposure, fever, or change in fetal movement. Ask a maternity clinician or pharmacist before using steroid creams, antihistamines, or antifungal products.

When to seek urgent advice

Contact your maternity team or clinician if the rash is spreading, persistent, very itchy, or affecting sleep. Seek urgent help for breathing difficulty, swelling of the lips or face, faintness, widespread blistering, skin peeling, purple spots, fever, or painful sores. Prompt maternity advice is also needed for itch on the palms or soles, yellowing of the skin or eyes, dark urine, or reduced fetal movement. These signs need assessment even if the rash seems mild.
