What should you know about IT band syndrome swelling?

Short answer
Visible swelling is not the defining feature of IT band syndrome, so swelling around the knee should be assessed rather than automatically attributed to it.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Understanding swelling
IT band syndrome usually causes pain and tenderness at the outer knee during repeated bending, running or cycling. A small area of irritation may feel puffy or sensitive after activity, but obvious, widespread or rapidly increasing swelling can suggest another knee problem, such as injury or inflammation inside the joint.
The position of swelling matters. Swelling behind or around the whole knee, bruising, warmth, restricted movement or a feeling of instability should be described during an assessment. A clinician may examine the knee and nearby hip and ankle rather than relying on the visible appearance alone.
What to do at home
Stop the activity that increases pain or swelling and avoid repeatedly bending the knee to check it. Elevate the leg when resting, and use a wrapped cold pack for short periods if that is comfortable and medically suitable for you. Do not apply ice directly to skin or use medication that is unsafe for your health conditions.
Once swelling and pain are settling, a physiotherapist can guide a gradual return to movement and strengthening. Gait retraining may help if walking or running mechanics are contributing. Massage over a visibly swollen or acutely painful area should wait until a clinician has advised that it is appropriate.
When swelling needs prompt care
Seek urgent medical care for a hot or rapidly swollen knee, fever, inability to bear weight, severe pain after an injury, a locked joint, marked redness, or new calf swelling. Get emergency help for chest pain or sudden breathlessness. If swelling keeps returning or does not improve with rest, arrange an examination.
Questions to ask
Ask where the swelling appears to come from, whether the pattern fits IT band irritation, what activities to avoid, whether medication or imaging is appropriate, and when physiotherapy or strengthening can safely begin.

Questions people ask
Does swelling confirm IT band syndrome?
No. Swelling can have several causes, and obvious swelling deserves an assessment rather than a self-diagnosis.
Can I massage a swollen knee?
Avoid direct massage over acute swelling until a clinician confirms that it is suitable and explains how to proceed.
