IT band syndrome pain is usually felt on the outside of the knee, sometimes extending upward along the outer thigh. People may point to the bony area just above the outer knee. Running, downhill walking, stairs, or repeated knee bending can bring it on. The pain may feel sharp, burning, or tight and can become more noticeable as activity continues. Pain at the front, inside, or deep within the knee may have a different cause, so location helps guide an assessment but does not prove a diagnosis.

Mapping the discomfort

The iliotibial band runs from the outer hip toward the outside of the knee. Irritation around its lower portion tends to stay near the outer knee, while altered movement may create tension in the outer thigh, hip, or calf. Note whether the pain is easy to locate with one finger, whether it appears only with movement, and whether it persists afterward. Swelling inside the knee, catching, locking, numbness, or pain that travels below the knee changes the picture and should be discussed with a clinician.

Sudden calf swelling, warmth, colour change, shortness of breath, or chest pain requires urgent help. Do not use a familiar outer-knee location to dismiss new or rapidly changing symptoms.
