What it can mean

The iliotibial band is a strong strip of connective tissue along the outer thigh. Repeated knee bending can irritate tissues near the outer knee, producing a focused ache or sharp pain that appears during a run and may ease after stopping. Downhill running, uneven surfaces, weak hip control, or a sudden training change can increase load. Knee pain has many possible causes, so locking, giving way, substantial swelling, or pain after a twist should not automatically be labelled IT band syndrome.

What you can do

Reduce running distance, speed, hills, and other movements that reproduce the pain. Keep comfortable low-impact activity if it does not worsen symptoms during or after exercise. Physiotherapy can build hip and leg strength, assess alignment, and adjust the return-to-running plan. Gait retraining may help if your stride or foot placement repeatedly increases outer-knee load. Sports massage can ease surrounding muscle tightness, but pressure directly over the painful area may irritate it. A cortisone injection is sometimes discussed when symptoms remain limiting after appropriate conservative care.

When to seek care

Arrange assessment if pain persists despite reducing provoking activity, prevents normal walking, or returns each time you resume running. Seek prompt care after a significant twist, a knee that locks or gives way, marked swelling, fever, redness, or inability to bear weight. Sudden severe pain needs evaluation rather than a return-to-sport test.
