Is neck dissection needed before radiotherapy?

A neck dissection removes selected lymph nodes and nearby tissue to treat or assess disease in the neck. Radiotherapy uses carefully planned radiation to damage cancer cells in a defined area. Some people have surgery first, some receive radiotherapy without neck surgery, and some have both in a sequence chosen by a head and neck cancer team. Decisions can reflect the primary tumour, scans, biopsy findings, surgical margins, swallowing function and previous treatment.

Ask the team to explain which lymph nodes are involved or at risk and what each treatment is intended to achieve. Discuss effects on shoulder movement, neck sensation, swallowing, saliva, skin and fatigue. A dental review, speech and swallowing assessment, nutrition advice or physiotherapy may be useful before treatment. Take the written plan to each appointment because surgery and radiotherapy teams may coordinate timing.

Contact the team urgently for new trouble breathing or swallowing, rapidly increasing neck swelling, uncontrolled bleeding, fever after surgery or inability to keep fluids down. Sudden weakness, severe chest pain or collapse needs emergency care.

Ask why surgery is or is not recommended, whether radiotherapy can be given first, how the plan affects swallowing and shoulder function, what recovery involves, and which symptoms should trigger a same-day call.
