Is mastectomy needed before lymphoedema therapy?

Lymphoedema is swelling caused by reduced movement of lymph fluid, often in an arm, breast or chest after cancer treatment. Therapy may include skin care, movement, compression and hands-on drainage techniques. A mastectomy removes breast tissue and is a separate operation; it is not a standard prerequisite for treating lymphoedema. Surgery may sometimes be discussed for breast cancer or for selected persistent swelling, but the reasons and timing are individual.

Arrange an assessment with a lymphoedema specialist or oncology team. They can check the swelling, skin, range of movement and possible causes, then tailor a plan. Tell them about breast surgery, lymph-node procedures, radiotherapy, infections, medicines and any change in swelling. Do not start tight compression or massage over painful, infected or newly operated skin without professional guidance.

Contact your clinical team promptly if swelling increases quickly, the skin becomes hot or red, pain develops, fluid leaks, or you feel feverish. Sudden breathlessness, chest pain or fainting needs emergency care. These symptoms can signal problems that need assessment before routine lymphoedema treatment.

Ask what is causing the swelling, whether you need imaging, which therapy is suitable now, how to protect your skin, and whether any breast or lymph-node surgery is actually being considered and why.
