Why running can hurt

IT band syndrome affects the tissues along the outer thigh and commonly presents with pain near the outside of the knee during repeated bending and straightening. A runner may notice symptoms after a change in distance, hills, speed, terrain or training frequency. The diagnosis should be checked because other knee, hip or lower-leg problems can feel similar.

Safety depends on how severe and irritable the symptoms are, whether walking is comfortable, and whether running mechanics or training load can be modified. Pain that settles quickly with rest may need a different plan from pain that persists at rest or alters your stride.

Managing a return to running

Reduce or pause the activity that provokes pain, then use low-impact movement only if it is comfortable. A physiotherapist can assess hip and leg strength, single-leg control, flexibility and training habits, then build progressive exercises. Resume running in small stages when everyday walking and the prescribed drills are tolerated without a worsening response.

Warm up, avoid sudden hill or speed increases, and keep a simple record of distance, terrain and symptoms. Supportive shoes may help comfort, but footwear alone will not correct every training or movement factor.

When to seek medical care

Arrange an assessment if outer-knee pain keeps returning, causes limping, prevents normal walking, or does not improve after reducing the aggravating activity. Seek urgent care after a major injury, for a locked or visibly swollen knee, inability to bear weight, fever with a hot joint, or new weakness or numbness.
