Is iron therapy safe in pregnancy?

Short answer

Iron therapy can be appropriate in pregnancy when a clinician recommends it for iron deficiency and chooses the dose and form for your situation.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why iron may be prescribed

Pregnancy increases the body's need for iron, and low iron can contribute to tiredness, breathlessness or anaemia. Treatment may involve an oral supplement, while an infusion may be considered if tablets are not tolerated, are not absorbed adequately or are not improving blood results. Safety depends on confirming the problem, selecting a suitable preparation and considering your trimester, allergies, other medicines and health history. Iron is not a general remedy for every type of anaemia, so testing helps guide treatment.

How to use it during pregnancy

Take only the preparation and dose prescribed for you, including any iron in a prenatal supplement. Tell your maternity clinician about all medicines and supplements, digestive symptoms and previous reactions. Ask how to reduce nausea or constipation and how to separate iron from medicines that affect absorption. If you receive an infusion, report discomfort or itching during treatment and follow the clinic's observation instructions. More detail is available in [Iron Therapy](/treatments/medical-gynaecology/iron-therapy/).

When to get urgent advice

Seek urgent help for trouble breathing, swelling of the lips or face, fainting or a severe reaction after taking iron or receiving an infusion. Contact your maternity team about persistent vomiting, severe abdominal pain, marked weakness, worsening breathlessness or bleeding. Do not stop prescribed treatment without discussing the reason, but report troublesome effects so the plan can be reviewed safely.

Questions for your maternity team

Ask which blood results support treatment, whether tablets or infusion are recommended, how the treatment affects your antenatal plan, when improvement will be checked and what side effects need a call.

Questions people ask

Should I take iron in pregnancy without testing?

Discuss it with your maternity clinician first, because anaemia has different causes and unnecessary iron may not address the problem.

Can iron therapy be given by infusion during pregnancy?

It may be considered in selected situations when an oral supplement is unsuitable or has not worked adequately.
