Is hip fracture surgery needed before a falls prevention programme?

Short answer
If a hip fracture is present, it needs medical assessment first; a falls prevention programme is usually introduced later when movement and weight-bearing are safe.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- First priority
- Assess and treat a suspected hip fracture
- Later focus
- Balance, strength, transfers and home safety
- Progression
- Exercises advance as weight-bearing becomes safe
How the two types of care fit together
A suspected hip fracture is an urgent injury. The immediate plan may include imaging, pain relief, temporary protection of the leg and, when appropriate, surgery to stabilise or replace damaged parts of the hip. A falls prevention programme has a different purpose: it addresses balance, leg strength, transfers, footwear, vision, medicines and hazards at home. It may begin during rehabilitation in a modified form, then progress as the person can safely practise movement.
Choosing the next step
Do not start unsupervised balance exercises after a new fall with hip pain or inability to stand. Seek medical assessment first. After the fracture plan is set, ask the rehabilitation team when exercises can begin and which activities are allowed. Keep walking aids adjusted correctly, remove trip hazards, improve lighting and make sure the person has support during transfers until the team confirms greater independence.
When urgent help is needed
Call emergency services after a fall that causes severe hip or groin pain, inability to bear weight, a shortened or turned-out leg, or new confusion. During recovery, seek prompt review for worsening pain, a new fall, wound drainage, fever, sudden weakness, chest symptoms or breathlessness.
Questions for your doctor
Has the hip been checked for a fracture? Is surgery needed, and what movement is safe while waiting? When can falls prevention training start, and which home changes would reduce immediate risk?

Questions people ask
Can falls prevention exercises begin before surgery?
Only if the treating team says they are safe. A suspected fracture must be assessed first, and exercise may be limited until the injury is stabilised.
Does surgery prevent future falls?
Surgery treats the fracture but does not remove every falls risk. Rehabilitation and a falls prevention programme address balance, strength, medicines and the home environment.
