Is closed reduction needed before physiotherapy?

Short answer

Closed reduction is needed before physiotherapy when a displaced fracture or dislocation must first be repositioned and stabilised; it is not a routine step for every rehabilitation plan.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why the order matters

Physiotherapy uses planned movement, strengthening and functional practice. If a bone or joint is not safely aligned, loading or moving it may worsen the injury. A clinician may therefore perform a closed reduction, immobilise the area and confirm the position before selecting rehabilitation exercises.

Reduction is not required for every person referred to physiotherapy. Sprains, stiffness, muscle problems and many post-operative plans may begin with assessment and gentle treatment without any reduction. Even after a reduction, therapy usually starts only when the treating clinician confirms that movement is safe for the specific injury.

Plan rehabilitation safely

Bring imaging reports, discharge instructions and the name of any splint or cast to your physiotherapy appointment. Do not ask a therapist to force a joint into position or begin weight-bearing before clearance. Your therapist can work within the medical restrictions, monitor swelling and gradually progress movement once stability and healing allow.

When to pause and seek care

Contact the treating team promptly if pain suddenly increases, the limb changes colour or temperature, numbness develops, swelling becomes rapidly tighter, or a splint or cast feels too restrictive. Seek urgent assessment after a new fall, visible deformity or inability to move the affected fingers or toes.

Questions for your doctor

Is my injury aligned well enough for therapy now? What movements and loads are restricted? Do I need repeat imaging before progressing? Which symptoms mean therapy should stop and the injury should be reviewed?

Questions people ask

Can physiotherapy realign a dislocated joint?

A physiotherapist should not independently force a dislocated joint; reduction needs assessment and treatment by an appropriately trained medical clinician.

Can therapy start after closed reduction?

Often it can, but the timing and exercises must follow the treating clinician’s restrictions.
