CLL can behave differently from one person to another. Some people are monitored without immediate treatment, while others need medicine because the disease is causing symptoms, blood-count changes or enlarged lymph nodes. Age alone does not decide the approach. The team also considers heart and kidney health, memory, mobility, nutrition, infection history, other medicines and the help available at home.

When treatment is needed, the clinician balances likely benefit with side effects and practical issues such as clinic visits, tablets, falls risk and interactions with existing medicines. A geriatric assessment or pharmacist review may identify ways to make treatment safer and easier to follow.

Bring a complete medicine and supplement list, including non-prescription products. Report infections, bruising, bleeding, night sweats, weight change, worsening tiredness or new lumps. Ask whether observation is appropriate, what monitoring involves, and how the plan fits with your priorities. A trusted relative or carer can join visits and help record instructions if you wish.

Contact the haematology team promptly for fever, chills, new breathlessness, unusual bleeding, rapidly worsening weakness, confusion, or signs of infection. Seek emergency help for severe breathing difficulty, collapse, uncontrolled bleeding or sudden one-sided weakness. Ask for clear after-hours instructions because infection and medicine reactions may need rapid assessment.
