What the markers can mean

A positive surface antigen can indicate current hepatitis B infection and usually needs clinical review and confirmatory testing. Surface antibody can indicate immunity, often after vaccination or recovery. Core antibody can suggest previous or current exposure because vaccination alone does not produce it. The exact pattern, timing, symptoms and laboratory ranges matter, and an isolated result may be unclear.

Further tests may include viral DNA, liver enzymes and repeat serology. Hepatitis B can occur without symptoms; when symptoms are present, fatigue or dark urine may be relevant but are not specific to this infection.

What to do with your result

Ask the clinician who ordered the test to explain each marker together, including whether the sample was taken during a recent exposure or after vaccination. Do not donate blood or share razors or toothbrushes until the result is clarified if current infection is possible. A clinician may arrange a liver function test, additional blood work, vaccination advice or specialist review. Household and sexual contacts may need testing or vaccination advice.

When to seek care

Seek urgent medical help for yellowing of the eyes or skin with confusion, severe vomiting, unusual drowsiness, bleeding, or intense abdominal pain. Arrange prompt review for new dark urine, pale stools, worsening fatigue or right-sided upper abdominal discomfort. A positive marker without symptoms still needs planned follow-up because infection can be clinically quiet.
