Internal bleeding in early pregnancy

Bleeding can arise from the cervix or vagina, a small area around the pregnancy, miscarriage, or an ectopic pregnancy. An ectopic pregnancy develops outside the uterus and may cause internal bleeding as it grows. Some people notice vaginal bleeding; others mainly develop pelvic or abdominal pain, often on one side, because blood is collecting internally. A clinician may use an ultrasound, examination, and blood tests to determine where the pregnancy is and whether it is developing as expected. The visible amount of blood cannot confirm how serious the cause is.

Contact your pregnancy care team or an urgent medical service as soon as bleeding starts, even if it is light. Record the timing, colour, flow, clots, and any pain. Mention your last menstrual period, pregnancy test result, prior ectopic pregnancy, fertility treatment, or surgery if relevant. Go to an emergency department or call emergency services for severe abdominal or pelvic pain, one-sided pain that is worsening, shoulder-tip pain, fainting, severe dizziness, unusual weakness, pale or clammy skin, confusion, or rapidly increasing bleeding. These symptoms can signal internal blood loss. Seek urgent help as well if you feel suddenly unwell without much vaginal bleeding. A routine appointment is not suitable for these warning signs.
