Intercostal muscle strain in the chest

Short answer
An intercostal chest strain affects a muscle between the ribs and often hurts with pressing the area, twisting, coughing, or taking a deep breath.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical trigger
- Overuse, twisting, lifting, impact, or repeated coughing
- Important distinction
- Chest pain with breathing problems needs prompt assessment
How a chest-wall strain presents
The intercostal muscles run between neighbouring ribs and assist breathing and trunk movement. Overuse, a sudden twist, forceful lifting, a blow, or repeated coughing can stretch these fibres. The discomfort is often localised and may feel sore, tight, or sharp when the rib cage moves. Tenderness over the muscle supports a chest-wall source, but it does not prove one.
Chest pain has many possible causes, including heart and lung problems. A strain should not be diagnosed from a single symptom, particularly when pain is sudden, severe, or unlike previous muscle pain. Changes in breathing, circulation, or general wellbeing deserve prompt attention.
Protecting the area
Pause heavy lifting, forceful twisting, and the activity that triggered symptoms. Gentle walking and comfortable breathing are preferable to complete bed rest. A wrapped cold pack may help early; later, warmth can relax guarded muscles. Do not strap the ribs tightly, and support the area with your hand or a pillow when coughing if that is comfortable.
As pain settles, restore movement gradually with easy shoulder and trunk motion before returning to sport or demanding work. A physiotherapist can assess breathing mechanics and provide a staged programme. Check with a clinician or pharmacist before using anti-inflammatory medicine if you have kidney, stomach, heart, bleeding, pregnancy, or medicine-related concerns.
Chest symptoms needing urgent help
Call emergency services for chest pressure or heaviness, breathlessness, fainting, cold sweating, blue lips, coughing blood, or pain travelling to the arm, jaw, or back. Seek same-day assessment for fever, a major blow, increasing swelling, worsening pain, or inability to take a normal breath. These signs require medical evaluation rather than home treatment for a strain.
What to ask
Ask whether the pain is consistent with a chest-wall strain, whether the ribs or lungs need checking, which analgesics are suitable for you, and when stretching, work, driving, or exercise can resume.

Questions people ask
Does tenderness mean the chest pain is definitely muscular?
No. Tenderness can support a chest-wall explanation, but it cannot exclude heart, lung, rib, or other causes.
Can I exercise with an intercostal strain?
Keep activity gentle and pain-limited at first, then build up gradually once breathing and everyday movements are comfortable.
