What should you know about intense hiccups?

Short answer
Intense hiccups usually settle with simple measures, but episodes that persist, recur frequently, or interfere with eating, sleeping, or breathing need medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What hiccups are
A hiccup is a sudden involuntary contraction of the diaphragm followed by closure of the vocal cords. Eating quickly, swallowing air, fizzy drinks, alcohol, temperature changes, laughing, or emotional excitement can irritate the reflex involved.
Persistent hiccups can sometimes accompany acid reflux, gastritis, irritation near the diaphragm, a medication effect, or another illness. The sound or intensity does not reveal the cause, so ongoing episodes need a clinician’s assessment.
What you can try
Pause eating and drinking, sit upright, breathe slowly, and take small sips of cool water. Eat slowly when you resume and avoid fizzy drinks, alcohol, very large meals, and sudden temperature changes. Do not attempt unsafe tricks that involve choking, extreme breath-holding, or swallowing objects.
Keep a note of when hiccups start, what preceded them, how long they last, and whether reflux, abdominal pain, vomiting, or medication changes occur at the same time. This helps a clinician look for a trigger.
When to seek care
Seek urgent help if hiccups occur with severe breathing difficulty, chest pain, confusion, fainting, new weakness, trouble speaking, repeated vomiting, or an inability to swallow fluids. Get medical advice when they prevent sleep, eating, drinking, or normal breathing.
Arrange a review for persistent or repeatedly returning hiccups, especially after starting a medicine or when you also have symptoms of acid reflux or gastritis. A clinician can decide whether examination or treatment is needed.
Questions for your doctor
Ask what may be triggering the episodes, whether reflux or stomach irritation should be assessed, whether any medicine could contribute, and which treatment is appropriate if simple measures fail.
